Candidates must be at least 16 years of age.
Candidates must possess the senior secondary school certificates or its equivalents (SSCE, NECO, NABTEB,GCE) with a minimum of five (5) credits in English language, Mathematics, Physics, Chemistry and Biology in not more than two sittings.
Birth certificates/declaration of age
Testimonial from the last school attended
A letter from your parish priest or pastor
Marriage certificate (if married)
Original/photocopy of relevant documents will be sighted during interview
METHOD OFAPPLICATION
Thrce (3) recent passport photographs
Baptismal card (very inmportant)
Bring any of PVC, NIN, International Passport, Drivers licence
Sale of form commences from 16th October 2023 to 6th December, 2023 at the school.
Form is obtained on payment of Non-refundable fee of (12, 550) into the school Account with the ACCOUNT NUMBER: 3031304862, Name of the account: Sehool of Midwifery Mater Hospital, Afikpo, First Bank Of Nigeria PLC.
Visit the school with the evidence of payment, change the bank teller to receipt and collect the entrance form.
Completed fons should be returned to school on or before 6th December, 2023
The courses for the entrance examination include: Mathematics, English Language,
Physics, Chemistry And Biology.
DATE OF EXAMINATIONS/INTERVIEWS: 7th & Sth December, 2023.
Candidates are to bring along with them the original receipt of payment. Only those who possess entry requirements and pass the entrance examination will be cligible for interview
